Overview
Our Community Social Development (CSD) Department is seeking two (2) results-oriented team members who demonstrate professionalism and a commitment to providing exemplary public service.

STATUS: Non-Permanent Term, up to 2 years (subject to continued external funding)
HOURS OF WORK: 37.5 hours per week
PAY RATE: $41.35 per hour
Working in partnership with Lethbridge Police Services and under the direction of the Community Social Development Operations Manager, the Safer Communities Case Manager is responsible for supporting youth at elevated risk of justice involvement, with an increasing focus on mental health, substance use, and complex social needs.
The Safer Communities program is evolving to align with emerging community trends and provincial priorities, with a stronger emphasis on integrated, prevention-focused approaches to reducing risk factors associated with criminal involvement. The Case Manager will lead intensive, individualized case management by addressing the root cause of risk through coordinated service planning, system navigation, and sustained engagement with youth and their support networks.
The Case Manager will oversee all expectations for service planning, including intake, admission, case plan development, evaluation, assessment of short- and long-term needs, referrals to appropriate resources, and exit planning. They will work in collaboration with Lethbridge Police Services and the Community Social Development team to ensure these outcomes are achieved.
In this role, you will:
- Operate within an evolving tiered case management model, delivering both short-term stabilization and intensive case management for youth at risk of or involved in the justice system, with a focus on mental health, substance use, and coordinated system supports.
- Complete intake for referrals provided by the Lethbridge Police Service.
- Conduct risk/need assessments and develop individualized service plans that are responsive to the needs of youth.
- Apply trauma-informed, strength-based, and person-centered approaches to support stability and long-term outcomes.
- Provide appropriate referrals to internal and external programs for long-term support.
- Support youth in building protective factors, including life skills, prosocial engagement, education, and community connection.
- Advocate for youth to access appropriate services and reduce barriers within complex systems.
- Engage and support families/guardians, where appropriate, to strengthen long-term outcomes.
- Use a strength based and person-centered approach to develop individualized service plans that address youth needs including health and emotional well-being, parental/guardian support, education, safety, recreational and community supports, legal and custody, and financial needs.
- Use creative case planning when developing life skills, meaningful daily activity, and connection to community.
- Ensure all documentation and client progress information is accurate and up to date as well as perform weekly maintenance of caseload files.
- Actively work with clients’ unique network of support by developing professional relationships with appropriate cultural or spiritual connections, family doctors, pharmacies, school districts, community resource centers and more to meet service plan goals.
- Initiate and support Case Conferences with client support systems as needed.
Qualifications & Experience:
- Bachelor’s degree in relevant disciplines is required with a focus on youth, public health, addiction and mental health, crime prevention, etc.
- Minimum of 3 years’ proven experience in results-oriented, high paced culture focused on youth and/or case management services.
- Proven track record in strategic social and/or health delivery case management, which could include clinical expertise relating to psychosocial support, addiction and mental health, or other social work.
- An understanding of youth care practice, crisis management and trauma-informed practice is considered an asset.
- Experience working with youth in the social services sector.
- Intensive case management experience.
- Strong communication skills; both written and verbal.
- Ability to work using trauma-informed and culturally respectful practices.
- Situational awareness and ability to assess and respond to safety risks.
- Ability to follow established procedures and work within defined scope of practice.
- Basic computer skills, including data entry and electronic documentation.
- Strong organizational skills and attention to detail.
Additional Information:
- This is a grant funded position and is subject to the continued receipt of funding from other orders of government.
- The successful applicant under consideration must have a valid Alberta class 5 driver’s license and a vehicle with appropriate auto insurance to provide transportation for programming participants.
- The successful applicant under consideration will be required to complete and pass a Police Information Check and Vulnerable Sector Check, Intervention Record Check, and an Extended Background Check conducted by Lethbridge Police Service.
